Directory of Records Index

Ontario's Freedom of Information and Protection of Privacy Act, R.S.O 1990 (FIPPA) requires the hospital to make available to the public an index of the general classes and types of records prepared by or in the custody or control of the University of Ottawa Heart Institute (Heart Institute).   

The Directory of Records index is organized into 8 functional categories.  Questions regarding the Directory of Records should be directed to the Freedom of Information Office (@email.

Personal Information Bank Index

Ontario’s Freedom of Information and Protection of Privacy Act, R.S.O. 1990 (FIPPA) requires the hospital to make available to the public an index of all personal information banks it maintains, including details on the categories of individuals about whom the information is maintained, the type of personal information maintained, where it is located, how it is used, by whom it is used, the legal authority for its collection and how long it is retained.

A personal information bank is personal information under the control of the institution that is organized or intended to be retrieved by an individual’s name or another identifier.

The Personal Information Bank index is organized according to the department or area within the hospital where the information resides. The directory is a working document and new entries are added as they are identified.
