Introduction
Congratulations on taking the first step to understanding your risk of cardiovascular disease. This risk assessment tool will ask you questions ranging from your family history to your own personal health. To complete the evaluation make sure to know the following: gender, age, diabetes status, smoking status, weight & height and systolic blood pressure. It is confidential, free and takes about 5 minutes to complete.
Once you complete the evaluation, you will be provided with a tailored summary report and receive recommendations on what you can do next to improve your health. If you have been diagnosed with cardiovascular disease, this tool will not accurately assess you risk. You should work with your doctor to determine your risk.
