How to Make an Information Request

Before you submit a written request for general records, please review the Directory of Records and Personal Information Bank page to see if the records you are seeking are included in our inventory. This will help you find records that are already public or to narrow down your request.  

All requests must be made in writing by completing an Access Request Form (pdf). The form can be emailed, mailed or submitted in person to the FIPPA office.

Fees

A $5.00 application fee is mandatory and non-refundable. A cheque or money order made out to “University of Ottawa Heart Institute” must be received by UOHI before processing of the request can begin. Payment can be sent by mail or delivered to the FIPPA office at the above address.  

Please note that your application will not be considered completed and will not be processed until the application fee is received by the FIPPA office.  

If the total cost of processing your request exceeds $100, you will be provided with a fee estimate before processing begins. You will be required to pay a deposit of 50% of the total processing fee before UOHI begins to process your request.  

Fees are charged on the following schedule:

Photocopies and computer printouts $0.20 per page
CDs $10.00 per CD
Manually searching for a record $30.00 per hour
($7.50 for every 15 mins spent by a person)
Preparing a record for disclosure, including severing part of the record $30.00 per hour
($7.50 for every 15 mins spent by a person)
Other charges, if incurred, will be provided in an invoice  

The requestor will receive a refund if the fee deposit paid exceeds the actual cost incurred by UOHI when processing the request.  

All fees are prescribed by Legislation.
