Ottawa Hospital multi-disciplinary Diabetes team provides integrated Diabetes self-care eudcation and medical management support to people with complex Diabetes care needs. Care is provided by an individualized, multi-disciplinary assessment and education; group education and follow up; and, integration of education with clinical management.

Hours: Mon-Fri 8 a.m to 5 p.m. Classes Wed &Thurs every other week
