Hours: Mon to Fri: 8:00 a.m. to 4:00 p.m. (closed between 12:00 p.m. and 1:00 p.m.; closes at 2:45 p.m. on Fridays)

Cost: $60.00 (Cash or cheque only)

Referral: Fax referral form to clinic and contact clinic for appointment.
